Notice of privacy practices

Our responsibilities

Better Place Now LLC is required by law to maintain the privacy and security of your protected health information, to give you this notice of our legal duties and privacy practices, to notify you promptly if a breach occurs that may have compromised your information, and to follow the duties and practices described in this notice.

How we may use and share your information

We may use and share your health information to treat you, including with other providers involved in your care; to run our practice, such as quality improvement and staff training; and to bill and get payment from insurance plans or other entities. Mental health records, psychotherapy notes, and substance use treatment records receive additional protections under federal and Arizona law, and we will obtain your written authorization before sharing them except where the law allows.

Other permitted uses

We may also share your information when required by law, for public health and safety purposes, to prevent a serious threat to health or safety, for health oversight activities, in response to a court order or subpoena as permitted by law, with a medical examiner or coroner, and for workers' compensation, law enforcement, or specialized government functions as the law permits.

Your rights

You have the right to get an electronic or paper copy of your medical record, to ask us to correct your record, to request confidential communications by a specific method or address, to ask us to limit what we use or share, to get a list of those with whom we have shared your information, to get a copy of this notice, to choose someone to act for you, and to file a complaint if you feel your rights are violated. You may file a complaint with us at the contact below or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

Your choices

For certain information you can tell us your choices about what we share, including sharing with family and friends involved in your care, and disclosures in disaster relief situations. We never share your information for marketing purposes or sell your information without your written permission.

Changes to this notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on request and on our website.
